Welcome to In Motion O.C.! You are about to begin the wonderful experience of healing. Many of your questions about what to expect are already answered in our FAQ page. As with anything else, there’s a little paperwork involved. Below are the options for paperwork:

• You may simply arrive 20 minutes early to your 1st appointment to fill out your paperwork.
• You may print out your paperwork here, fill it out at home, and fax it to us at (949)861-8601. If you don’t have access to a fax machine, then bring it in with you to your first appointment.

Our phone number is (949)861-8600 and here are our Driving Directions. Please wear comfortable clothing (t-shirt, shorts or sweats), and exercise shoes to your first appointment.

Payment Procedures
Your copay is due at the time of your visit unless other arrangements are made in advance. We accept the following payment types: Visa, MasterCard, Cash, Check and Money Order.

For a list of commonly accepted insurance policies visit our insurances page. It is our policy to bill your insurance for your visits. You are, however, responsible for paying any portion not covered by your insurance policy.
